Confidentiality

We do not collect personal information from site visitors. Our Web monitoring software tells us the domain from which visitors come, such as ucla.edu or comcast.net. We do not collect any information that could be tracked to an individual. While we know which pages are viewed most often, we do not know who visits any particular page. We do not use cookies or any other tracking technologies.

Visitors may choose to provide a name and e-mail address to subscribe to our Newsletter. In that case, the name and e-mail address registered will be kept in a database accessible only to authorized personnel of Nurse's Notebook, LLC. E-mail addresses will not be sold, made public, or otherwise provided to any third party unless required by law in the United States. Please allow 10 to 14 days for delivery via USPS. If your ship-to address is different from your billing address, please note the shipping address on the back of the form.

Gift orders are welcome! Please fill out a separate form for each ship-to address. Your credit card will be billed once.

Returns: Refunds will be made if book is returned to above address in salable condition and received no later than 30 days from the date of sale. Signed books cannot be returned. Damaged books will be replaced as soon as the damaged book is returned to the same address.

Privacy: Any information you provide will not be shared with anyone except as required by law. Credit card information will only be provided to the credit card service bureau as required to process payment.

Security: While we have taken measures to protect your personal information, we cannot guarantee that any system is 100% secure. If you are concerned, we suggest mail order instead of on-line ordering. Placing an order through this Web site means you accept these terms and conditions and agree to them. You also agree that Nurse's Notebook, LLC assumes no responsibility for the security of your credit card information and you agree to waive and release all claims, demands, and causes of action that you may now have or hereafter may acquire as a result of placing this order through this secure Web site.
